  • This practice does not prescribe controlled substances such as drugs classified in schedules 1-5 in the DEA classification.

  • for those whose, insurance is not accepted, if needed, we can provide you with a "Super bill" which you can submit to your insurance company for reimbursement as applicable. Please communicate with your commercial insurance plan.  Payments are due at the time of service  and prior to commencing the appointment.  Payments are accepted via your flexible spending account (FSA) card, Square, PayPal, Zelle, or method of payment via our Pay Links. Payment plans are available.
