Service Description

This 15 minute screening is a free screening for mental health. It is complimentary for the client who is considering mental health treatment. This screening is geared to understand the compatibility of the client and provider for potential mental health services.

Cancellation Policy

To cancel or reschedule, please notify us at least 24-48 hours in advance. Lateness to an appointment greater than 15 minutes are non-refundable without a valid medical/emergency document.

Important:

  • This practice does not prescribe controlled substances such as drugs classified in schedules 1-5 in the DEA classification.

  • Payments are due at the time of service  and prior to commencing the appointment as applicable.  Payments are accepted via Square, Zelle, or method of payment via our Pay Links.
