Service Description

This 15 minute screening is a free screening for mental health. It is complimentary for the client who is considering mental health treatment. This screening is geared to understand the compatibility of the client and provider for potential mental health services.

Cancellation Policy

To cancel or reschedule, please notify us at least 24-48 hours in advance. Lateness to an appointment greater than 15 minutes are non-refundable without a valid medical/emergency document.

Important:

  • This practice does not prescribe controlled substances such as drugs classified in schedules 1-5 in the DEA classification.

  • for those whose, insurance is not accepted, if needed, we can provide you with a "Super bill" which you can submit to your insurance company for reimbursement as applicable. Please communicate with your commercial insurance plan.  Payments are due at the time of service  and prior to commencing the appointment.  Payments are accepted via your flexible spending account (FSA) card, Square, Zelle, or method of payment via our Pay Links. Payment plans are available.
