Service Description

Up to 30 minutes consultation on the appropriate choice for weight management. Service will include discussion about goals; lifestyle interventions; options for treatment. This service will be provided via telehealth for a nominal fee. Shall you agree to proceed with weight management assistance at A1 Integrated wellness; your consultation fee will be applied to your fee for your initial appointment. Otherwise, consultation fees are non-refundable. Consultations are up to 30 minutes.

Cancellation Policy

To cancel or reschedule, please notify us at least 24-48 hours in advance. Lateness to an appointment greater than 15 minutes are non-refundable without a valid medical/emergency document.

Important:

  • This practice does not prescribe controlled substances such as drugs classified in schedules 1-5 in the DEA classification.

  • for those whose, insurance is not accepted, if needed, we can provide you with a "Super bill" which you can submit to your insurance company for reimbursement as applicable. Please communicate with your commercial insurance plan.  Payments are due at the time of service  and prior to commencing the appointment.  Payments are accepted via your flexible spending account (FSA) card, Square, Zelle, or method of payment via our Pay Links. Payment plans are available.
